Sun Pharma gets exclusive rights to manufacture and commercialise the once-monthly PCSK9 inhibitor across markets outside the US and China

Sun Pharmaceutical Industries has entered into an exclusive licensing agreement with US-based LIB Therapeutics to manufacture and commercialise lerodalcibep across markets outside the United States and China, expanding the Indian drugmaker's presence in the global cardiovascular and lipid-management segment.

Under the agreement, Sun Pharma will hold exclusive rights to commercialise and manufacture the drug in the licensed territories. The company will also be responsible for pursuing regulatory approvals in markets where lerodalcibep has not yet received approval. Financial details of the transaction, including the upfront payment, milestones and royalties payable to LIB Therapeutics, were not disclosed.

What Is Lerodalcibep?

Lerodalcibep is a once-monthly PCSK9 inhibitor designed to reduce low-density lipoprotein cholesterol, commonly known as LDL or “bad” cholesterol.

PCSK9 inhibitors work by blocking the PCSK9 protein, helping the liver remove more LDL cholesterol from the bloodstream. The treatment is intended for adults with hypercholesterolaemia and related cardiovascular-risk conditions who require additional LDL-C reduction.

The US Food and Drug Administration approved lerodalcibep, marketed in the US as LEROCHOL, in December 2025. The FDA describes it as a once-monthly subcutaneous injection used alongside diet and exercise to reduce LDL-C in adults with hypercholesterolaemia, including heterozygous familial hypercholesterolaemia.

Once-Monthly Dosing Could Support Patient Convenience

One of the notable features of lerodalcibep is its once-monthly dosing schedule.

The drug's dosing frequency provides an alternative to daily oral lipid-lowering treatments and could be relevant for patients who require additional LDL-C reduction despite existing therapies.

In the pivotal clinical programme, patients received lerodalcibep every four weeks. The FDA's review covered trials involving adults with atherosclerotic cardiovascular disease or elevated cardiovascular risk, as well as patients with heterozygous familial hypercholesterolaemia.

Clinical Data Support LDL-C Reduction

Clinical development of lerodalcibep has shown substantial reductions in LDL cholesterol.

LIB Therapeutics reported that its Phase 3 programme demonstrated sustained LDL-C reductions of at least 60 per cent in patients with cardiovascular disease or high cardiovascular risk and at least 50 per cent among patients with heterozygous familial hypercholesterolaemia. The global Phase 3 programme enrolled more than 2,900 patients across its registration-enabling studies.

The FDA's review also found the treatment effective in reducing LDL-C in the studied patient populations, supporting its approval in the United States.

Sun Pharma to Handle Global Expansion Outside US and China

The licensing agreement gives Sun Pharma access to a broad international territory, excluding the United States and China.

This means Sun Pharma can pursue commercial opportunities across multiple regions where the treatment is approved or where regulatory applications can subsequently be submitted.

The company will also be responsible for seeking regulatory approvals in markets where lerodalcibep is not yet approved, making regulatory execution an important component of the partnership.

Cardiovascular Market Provides a Large Opportunity

High LDL cholesterol is a major risk factor associated with cardiovascular disease, creating a substantial global market for lipid-lowering medicines.

PCSK9 inhibitors occupy an advanced segment of this market and are generally used when additional LDL-C reduction is required, including in patients who do not achieve sufficient reduction through other treatments.

The availability of a once-monthly treatment gives Sun Pharma an opportunity to build a presence in this segment through its international commercial and regulatory infrastructure.

Agreement Adds to Sun Pharma's Specialty Portfolio

The transaction is consistent with Sun Pharma's broader focus on expanding its specialty and innovative medicines portfolio.

The company has increasingly sought to complement its established generics business with differentiated therapies that can provide higher-value opportunities in international markets.

Adding lerodalcibep to its portfolio gives Sun Pharma another product in a specialised cardiovascular segment, although the ultimate commercial contribution will depend on regulatory approvals, pricing, market access and physician adoption.

LIB Therapeutics Retains US and China Markets

The agreement specifically excludes the United States and China.

LIB Therapeutics retains the rights to those markets, while Sun Pharma receives the commercialisation and manufacturing rights for the remaining territories.

The structure allows both companies to pursue their respective geographic strategies while combining LIB's development of lerodalcibep with Sun Pharma's international commercial infrastructure.

Regulatory Execution Will Be Key

Although lerodalcibep has received US regulatory approval, the regulatory position can differ across individual markets.

Sun Pharma will therefore need to pursue approvals wherever the product has not yet been cleared for marketing. The speed of these regulatory processes will influence how quickly the company can introduce the product across its licensed territories.

Regulatory requirements, reimbursement frameworks and market-access negotiations will also influence the drug's commercial potential.

Implications for Sun Pharma

The agreement provides Sun Pharma with another potential growth avenue in the global specialty pharmaceutical market.

The company can leverage its international presence to introduce the therapy across multiple territories, while its manufacturing and commercial capabilities could support the product's eventual scale-up.

However, the financial impact of the agreement will depend on the pace of launches, regulatory approvals, pricing, competition within the PCSK9 market and patient adoption.

Market Outlook

Sun Pharma's licensing agreement with LIB Therapeutics expands its portfolio into the global PCSK9 inhibitor market through a once-monthly cholesterol-lowering treatment. The deal provides exclusive rights outside the US and China and adds another specialty product to the company's international growth pipeline.

The key factors to monitor will be regulatory approvals in individual markets, launch timelines, pricing and reimbursement, physician adoption and the competitive landscape for LDL-C-lowering therapies.

For investors, the agreement represents a potential additional growth driver, but its eventual contribution to Sun Pharma's financial performance will depend on successful regulatory and commercial execution across the licensed territories.
