Rating agency expects stronger organic growth, improving profitability and higher cash accruals as Metropolis benefits from network expansion, better test mix and the turnaround of Core Diagnostics
Crisil Ratings has revised its outlook on the long-term bank facilities and non-convertible debentures of Metropolis Healthcare Ltd (MHL) to ‘positive’ from ‘stable’, while reaffirming the company’s long-term rating at ‘Crisil AA-’. The rating agency also reaffirmed the short-term bank facility rating at ‘Crisil A1+’.
The outlook revision reflects expectations of an improvement in Metropolis Healthcare’s organic revenue growth, profitability and cash generation. Crisil also highlighted the anticipated turnaround in Core Diagnostics Pvt Ltd, which the company acquired as part of its strategy to strengthen its presence in the diagnostics market.
Revenue growth remains strong
Metropolis Healthcare reported a 24 per cent increase in revenue to ₹1,646 crore in FY26, according to Crisil. The growth was supported by higher patient volumes as well as increasing demand for specialised diagnostic and wellness tests offered under the company’s TruHealth brand.
The growth momentum continued into FY27, with revenue rising 16.6 per cent year-on-year in the first quarter.
The performance indicates that the company's growth is being supported not only by higher testing volumes but also by a gradual shift towards specialised and higher-value diagnostic services.
Expanded laboratory network could improve utilisation
Crisil expects better utilisation of Metropolis Healthcare’s expanded laboratory network to support operating performance over the medium term.
Higher utilisation can allow the company to spread fixed laboratory and infrastructure costs across a larger number of tests. Combined with an improved test mix, this could support operating-margin expansion if the company continues to maintain volume growth.
The rating agency expects these factors to contribute to healthier operating margins over the medium term.
Core Diagnostics turnaround becomes important
The performance of Core Diagnostics is another key factor behind Crisil’s positive outlook.
Metropolis has been working to improve the acquired business and integrate it into its broader diagnostics network. Crisil expects improved profitability at Core Diagnostics to contribute to the company's overall financial performance.
A successful turnaround could also strengthen Metropolis Healthcare’s presence in specialised diagnostics and improve the contribution of higher-value tests to the overall business.
The pace of improvement in Core Diagnostics, therefore, will remain an important operating metric for the company.
North India presence expands
Recent acquisitions have also strengthened Metropolis Healthcare’s geographical footprint, particularly in northern India.
According to Crisil, the contribution of North India to the company's domestic revenue has increased to around 17 per cent from 9 per cent.
The expansion gives Metropolis greater access to regional markets and can potentially improve the utilisation of its diagnostic infrastructure. It also reduces the company's dependence on its traditionally stronger markets and broadens its domestic revenue base.
Strong balance sheet supports expansion
Crisil highlighted Metropolis Healthcare’s robust financial risk profile and strong liquidity position.
As of March 31, 2026, the company's adjusted net worth stood at approximately ₹1,335 crore, while total debt was around ₹232 crore. A significant portion of this debt primarily comprised lease liabilities, while bank borrowings remained limited.
The relatively modest leverage provides the company with financial flexibility as it invests in laboratory infrastructure, technology, network expansion and potential acquisitions.
Cash generation provides capex support
Crisil expects annual cash accruals of approximately ₹330-370 crore.
The rating agency expects these cash flows, together with the company's liquid surplus, to be sufficient to fund regular capital expenditure estimated at around ₹65-85 crore over the near-to-medium term.
Strong internal cash generation reduces the company's dependence on external borrowing for routine expansion and maintenance expenditure.
It also provides additional flexibility to pursue selective acquisitions, provided such investments remain within manageable financial-risk parameters.
Diagnostics industry remains competitive
Despite the positive outlook, Metropolis operates in a highly competitive diagnostics market.
The sector includes large organised diagnostic chains, regional laboratories, hospital-based diagnostic centres and an increasing number of technology-enabled testing platforms. Price competition can therefore remain intense, particularly in routine diagnostic tests.
Crisil noted that moderate entry barriers and intense competition partially offset the company's strengths, including its established market position and operating efficiency.
The ability to maintain pricing discipline while continuing to increase test volumes will therefore remain important for sustainable margin improvement.
Specialised diagnostics provide an additional growth opportunity
The increasing demand for specialised diagnostics and preventive healthcare could provide another avenue for growth.
Metropolis has expanded its portfolio beyond conventional pathology testing through specialised diagnostics and wellness offerings. Premium health-check packages and advanced testing can potentially increase average revenue per customer while strengthening the company's positioning in preventive healthcare.
However, growth in these categories will depend on consumer adoption, pricing, physician referrals and the company's ability to maintain differentiated service quality.
Acquisitions remain part of the growth strategy
Metropolis Healthcare has increasingly used acquisitions to expand its geographical presence and strengthen its diagnostic capabilities.
The strategy can accelerate market expansion, but successful integration remains critical. Acquired businesses need to achieve expected revenue growth and profitability for acquisitions to create sustainable value.
The expected improvement at Core Diagnostics is therefore particularly relevant to the company's broader acquisition strategy.
What investors will track
Following Crisil’s positive outlook revision, key operating indicators for Metropolis Healthcare will include organic revenue growth, patient volumes, average revenue per test, specialised-test contribution, laboratory utilisation and operating margins.
Investors will also be watching the pace of improvement in Core Diagnostics, the contribution from northern India and the company's approach towards future acquisitions and capital expenditure.
At the balance-sheet level, maintaining moderate leverage and strong cash generation will remain important as the company continues to expand its network.
Market Outlook
Crisil’s revision to a positive outlook reflects expectations of improving operating performance rather than a change in the company's existing ‘Crisil AA-’ long-term rating. The reaffirmation of the ‘A1+’ short-term rating also reflects the rating agency’s assessment of the company's strong financial position.
Metropolis Healthcare enters the coming periods with several potential growth drivers, including healthy patient volumes, expansion in North India, greater laboratory utilisation, specialised diagnostics and the expected turnaround of Core Diagnostics.
At the same time, competition, pricing pressure, acquisition integration and execution remain important factors. The combination of revenue growth, operating-margin improvement and strong cash generation will be key indicators in determining whether the company can convert its expanding network and diagnostic portfolio into sustained financial performance.
